TRANSPORTATION

Focus Forward Clients

Transportation services will only be available to Focus Forward clients to transport them from their place of residence to the program only. We currently do NOT offer services to places appointments or personal trips. 

Payments Accepted

Transportation services can be paid through MedWaiver, the Developmental Disabilities Managed Care Pilot Program, or self- pay. If you have questions about funding, don't hesitate to reach out to Miranda Horr, the supervisor of operations with your questions!

Monday-Friday

Transportation services will be offered to clients in accordance with Focus Forward's schedule, Monday-Friday. In order to get to Focus Forward by its start time, pickups and drop-offs will comply with the program's 9am-3pm hours.

Expectations of Services

Transportation services will be billed for each week, so payments are expected to be made by the end of the next week, if self-pay. Clients are expected to be able to get on and off the van without assistance. Clients are also expected to be ready within 5 minutes of the van arriving.

Door to Door

Transportation services will only offer transport from client's homes directly to Focus Forward, and back. Driver will ensure the clients get picked up and dropped off with staff  supervision and with clear communication with families.

Application Process

If your young adult is interested in receiving transportation services, first fill out the application. If your young adult qualifies for services, support coordinators, care coordinators, or guardians will be contacted to set up payment information. Then the last step will be to fill out and sign the registration packet in order to begin receiving services.
